What is the process of obtaining a patient portal account?
First, ALL patients must obtain a temporary Password to protect their identity.
Please contact your Doctor’s office to verify your information. A temporary password will be provided to you and a notification will be sent to your email on file. Follow the link to the Registration process. This will allow you to create your login ID and a password of your choosing. Once that is done, you will be able to access the online patient portal.

**If you have already registered for a patient portal account but cannot view your clinical information, you must contact your Doctor’s office for help with this process. 

My password is not working. How can I reset my password?
Underneath the login field, select “I forgot my password” and answer the two security questions you submitted during your registration. If you don’t remember, please contact us.

I am receiving an Account not verified. This page is not accessible...” message.
This message means you did not verify your identity. Contact your Doctor’s office to verify your information. A notification email will be sent to your email on file. Follow the link in the email and follow the instructions to log into the patient portal.
